Case follow-up opens “Follow-ups for related contacts” instead of the Case Follow-Up form in Go.Data 2.50.3

Bug description
When trying to create a follow-up for a case, Go.Data opens the Follow-ups for related contacts / Create follow-up page instead of the Case Follow-Up form.

To Reproduce

Scenario 1

1. Go to Cases
2. Select a case
3. Click on Add follow-up
4. See that the application opens ... / Follow-ups for related contacts / Create follow-up instead of the case follow-up form

Scenario 2

1. Go to Cases
2. Select a case
3. Click on View follow-ups
4. Click on Add
5. See that the application opens ... / Follow-ups for related contacts / Create follow-up instead of the case follow-up form

Expected behavior
Creating a follow-up from a case should open the Case Follow-Up form for that case.

Additional context

• Build: 2508071107
• This happens in a custom tuberculosis outbreak configuration
• Case follow-up is enabled
• A case follow-up template is configured
• A contact follow-up template is also configured separately
• The issue happens both:
  • directly from the case action Add follow-up
  • and from View follow-ups > Add
• The behavior suggests that case follow-up creation is incorrectly routed to the related contacts follow-up creation page
